1、 Etiology of male pseudohermaphroditism

The contradiction between morphological standards is abnormal sexual differentiation or hermaphroditism, which can be divided into sex chromosome aberration, male pseudohermaphroditism, female pseudohermaphroditism, and true hermaphroditism. Among them, male pseudohermaphroditism refers to one of the most complex causes and types of abnormal sexual differentiation, with a chromosome of 46XY and a gonad of the testicles, but no masculinity in the reproductive tract and/or external genitalia. The etiology of this disease can generally be found in congenital testicular hypoplasia syndrome, reductase deficiency, pararenal hormone deficiency, and testosterone synthesis disorders. The occurrence of male pseudohermaphroditism. This situation may be a congenital developmental malformation of the reproductive organs, which can be treated through surgery. Pseudohermaphroditism refers to the presence of male genital testicles in the body, but similar in appearance to female external genitalia, but without menstruation.

If male pseudohermaphroditism occurs now, it should be considered to be caused by chromosomal abnormalities. In this case, consideration should be given to the possibility of poor quality or lack of sperm. Pseudohermaphroditism can directly affect the fertility of patients, leading to infertility. The testicle is the main reproductive organ of men, and its important function is to produce sperm that can combine with the egg to form a fertilized egg, which is a necessary condition for the birth of new individuals. Otherwise, women are unlikely to conceive. So whether it is acquired or congenital, it is infertility.

3、 Can male pseudohermaphroditism be cured?

The treatment of hermaphroditism can be cured. The younger the better. Generally speaking, gender should be defined between the ages of 2 and 3. Gender orientation should be determined comprehensively based on social gender, psychological gender, physiological gender, parents' and patients' own wishes. The surgical treatment of hermaphroditism mainly involves plastic surgery of the external genitalia, including penidoplasty, vaginoplasty, hypospadias repair, clitoral labioplasty, external vaginal opening widening, and other surgeries. Surgical treatment of male pseudohermaphroditism: Male external genitalia should be shaped according to male gender, with only penile straightening and urethroplasty required. The female internal genitalia on the cryptorchidism side should be removed without affecting the male reproductive tract. If the external genitalia resembles male or non male types, gender should be determined based on the development of the penis and testicles, and the external genitalia should be shaped according to male or female characteristics. Patients with testicular feminization undergo vaginoplasty after removing both testicles. Female pseudohermaphroditism with clitoral hypertrophy should be treated with clitoral reduction and plastic surgery as soon as possible. The ventral part of the hypertrophic clitoris was mostly removed, leaving only one-third of the clitoral tissue on the dorsal side and the associated neurovascular bundles, which were reshaped and sutured to form the clitoris. Patients with narrow vulvar meatus underwent an enlarged surgery, where the upper vaginal opening was opened and expanded, and the wound was repaired by transferring and inserting a lip flap or pudendal sulcus flap. If the true hermaphroditism is male, the ovarian tissue should be removed, along with the removal of the fallopian tubes, uterus, and enlarged breasts. After removal, the vaginal mucosa should be closed. As a woman, it is necessary to remove testicular tissue and undergo plastic surgery on the enlarged clitoris.

Pseudohermaphroditism is a relatively complex congenital disease, but it can be properly resolved through surgery. However, the disease is relatively difficult to treat in adulthood, but it is easy to treat in early childhood. With some minor surgeries, it can return to a normal person's appearance, and it will not have any impact on continued fertility in the day after tomorrow. But if it's an adult, the treatment needs to follow the doctor's advice!
